REQUEST TO TRANSPORT STUDENT FROM GAME SITE OR ACTIVITY

(For use with students riding district transportation to attend an activity, but wish to return home with a parent/guardian.)
Please make arrangements to have this form completed 48 hours prior to the scheduled trip.

I understand and acknowledge that athletes are expected to ride the scheduled school transportation to and from the event. I do not wish to have my student use district transportation to return home from this activity and request that my student be released to my custody. I understand that my signature signifies that USD 443 is no longer responsible for the safety or transportation of my student on his/her return home from this event.
